Paradigm is seeking a compassionate and organized LPN Post-Acute Care Coordinator to support injured workers through the coordination of post-acute care services. In this fully remote role, you'll collaborate with healthcare providers, case managers, and insurance professionals to facilitate care transitions, coordinate services, and ensure positive outcomes for injured workers. This is an excellent opportunity for a detail-oriented nurse who enjoys building relationships, solving problems, and making a meaningful impact in the recovery journey of injured workers.This is a fully remote, full-time position working Monday through Friday from 12:00 PM to 8:30 PM Eastern Time. The hourly pay range is $28.00 to $30.00. Responsibilities: Process referrals for injured workers for the Post-Acute Care Department including obtaining all clinical information needed from hospitals, physicians, case managers, etc. Provide staffing for Post-Acute Care referrals. Ensure appropriate communication is delivered on a consistent basis with clients regarding progress of their injured workers. Accurately addresses questions and inquiries from Adjusters, Case Managers, Providers, and Injured Workers in a timely, professional manner. Obtain and review orders received. Collaborate with client case managers and insurance carriers case managers to assist in placing injured workers in appropriate facilities for care. Identify and contact providers both in network and out of network. Follow up with healthcare facilities and providers to monitor progress and regularly obtain medical notes as needed. Coordinate discharges with applicable facilities. Facilitate any other services the injured worker may need post discharge. Positively support the mission, values, and vision of Paradigm through effective communication with internal and external customers, serve as positive role model, demonstrate excellent customer service, and comply with all applicable regulatory requirements and Quality Assurance initiatives. Consistently maintain a high level of ethical and professional standards. Provide appropriate follow up with supervisor, co-workers, case managers, and providers regarding updates, reported complaints and concerns. Identify any concerns within the department and bring to the attention of the Director of Post-Acute Care. Collaborate effectively with all departments through interdepartmental referrals and communication. Maintain reliable and predictable attendance during scheduled work hours Demonstrates a customer-first mindset by developing a broad and deep (where appropriate) understanding of Paradigm organization, products, operations, and customers. Prioritizes collaboration to meet customer needs and expectations and takes personal accountability for service quality. Support and participate in common teamwork to achieve departmental and organizational goals, cooperate, and work together with team as well as other internal and external partners. Actively participate in the QA process and departmental improvement efforts. Provide a work environment free from all illegal and discriminatory behavior and harassment, including sexual harassment. Utilizes AI tools to support day-to-day tasks, improve efficiency, and enhance output quality. Adopts new technologies as trained and applies them in alignment with established processes and guidelines. Responsible for complying with Paradigm Information Security requirements and policies, for safeguarding Paradigm or Paradigm related passwords, and for notifying Paradigm of any Information Security incidents per policy SEC 10-12 Information Security Incident Management. Qualifications: Education/Training: Current LPN state license in good standing with the State Board of Nursing. Experience: Minimum of one (1) year of nursing experience. Knowledgeable of injured worker’s rights and ability to ensure an atmosphere which to promote the privacy, dignity, and well-being of injured workers. Maintain confidentiality of all data, including injured workers’ Protected Health Information (PHI), employee, and operations data. Skills: Excellent verbal and written communication, cooperation, service excellence, and effective conflict resolution skills; Able to promptly address concerns with tact and provide appropriate follow up; Service Excellence; proficient reading, writing, grammar, and mathematics skills; strong knowledge of medical terminology; Excellent organizational, prioritization, and multi-tasking skills; Excellent computer skills (e.g., Outlook, Teams, to be trained on proprietary software).
